After nearly 50 years of hiding, Leatherface returns to terrorize a group of idealistic young friends who accidentally disrupt his carefully shielded world in a remote Texas town.
In the 2022 sequel/reboot of The Texas Chainsaw Massacre, Melody is a young entrepreneur from San Francisco. She has a plan to revitalize a near-ghost town in rural Texas into a tourist and real estate destination for wealthy elitists. She drags along her younger sister Lila to a meeting with potential investors in the town. Lila is slowly recovering from the shocking and traumatizing shooting at her school, and Melody sincerely desires for her sister to come live with her in Texas so that they can remain close together during this difficult time.
Melody, Lila, and their friends are alarmed to see a Confederate flag flying from a nearby house when they arrive in Harlow, Texas, which they don't want their potential investors to see. They try to remove it but are confronted by an older woman who lives there. She asks them what they are doing in her house. The older woman was pleased to be able to offer her guests some sweet tea. She explained that her house had been an orphanage, which she had run for close to 50 years. When Melody tries to tell the older woman that the bank has reclaimed their house and she needs to leave, the woman gets upset. While she's panic-stricken, a scary figure appears in the stairwell. Unfortunately, we can't see his face. The older woman tells the man to go back to his room, that she is doing well.
Melody explains that he is "the last of my boys" and that he needs special care and can't go outside, so she can't be here. The authorities come to take the older woman away, and she collapses in fear. The figure from before places her in the police cruiser to be taken to the hospital. One of Melody's friends offers to ride with them to the hospital so that she can reassure Melody that the woman is okay. As they drive away, Lila sees the looming figure's face in shadow, and she catches only an eye.
That menacing figure is none other than Leatherface, the chainsaw-wielding murderer Gunnar Hansen first played in the original 1974 Texas Chainsaw Massacre film. When the orphanage owner passes away en route to the hospital, Leatherface goes on a murderous rampage, using his chainsaw to kill everyone in the vehicle.
The rest of the movie is mainly Leatherface hunting down Melody, who he holds responsible for the death of his mother-figure. The only person left alive from Leatherface's murderous rampage 50 years ago, Sally Hardesty, hears that he is back. She visits Harlow with armaments, hoping to end him permanently.
Leatherface cuts a lot of people with his chainsaw, and only Melody and Lila are left alive. Sally arrives in her van and demands that they stay until she can kill Leatherface. Unfortunately, things turn for the worse for Sally when she nearly gets killed by Leatherface. He then goes after Melody and Lila. The two try to hit him with their van but end up crashing instead. Melody is stuck in the van, and she tells her sister to escape without her. Lila does so—but then comes back and attempts to shoot Leatherface. Unfortunately, she's out of ammo, but luckily, Sally appears and uses her last strength to shoot Leatherface in the shoulder.
