Set in 1998, this origin story explores the secrets of the mysterious Spencer Mansion and the ill-fated Raccoon City.

Claire Redfield and her older brother Chris live at the Raccoon City Orphanage. Claire becomes friends with Lisa Trevor, a girl disfigured by Dr. William Birkin, an Umbrella Corporation employee, who oversees the orphanage and abducts children for experimentation. Claire is selected to participate in an experiment, escaping Birkin.
Claire returns to Raccoon City in 1998, hitchhiking in a semi-truck. Unfortunately, the driver hits a woman on the road accidentally. As the driver and Claire argue, the woman mutates into a zombie and vanishes. The truck driver's Dobermann licks the blood the woman left behind and begins to foam at the mouth and behave erratically as time passes. Meanwhile, rookie police officer Leon S. Kennedy is eating in a diner and notices blood coming out of the eyes of the owner.
When Claire arrives at Chris' home to warn him of Umbrella's experiments, disclosing Ben Bertolucci as her information source, Chris leaves to warn the police station. A child running from his mother breaks into the house and attacks Claire; both mother and child are erratic, sickly, bloody, and have severe hair loss. Claire borrows her brother's motorbike and escapes.
Chief Brian Irons meets with the STARS Alpha team at the police station, who explains that the Bravo team went missing at the remote Spencer mansion while investigating a death. The Alpha team, composed of Richard Aiken, Chris Redfield, Jill Valentine, Albert Wesker, and Brad Vickers, is sent by helicopter to investigate the mansion, which they discover to be plagued by mutated zombies.
Wesker, unknown to his teammates, is an operative for an unidentified party tasked with acquiring Birkin's virus, navigating the mansion using inside information. After a zombie bites the pilot, the helicopter crashes into the manor. Richard and Chris encounter zombies eating the bodies of the Bravo team. While Chris battles the horde, a zombie devours Richard. Chris and Jill reunite and flee into the hidden route Wesker opened.
At the police station, his dog bites the truck driver, who mutates into a zombie and crashes his truck into the building. Umbrella guards straining to contain the outbreak stop Chief Irons from trying to drive out of the city while shooting civilians attempting to escape. When Iron returns to the station and is attacked by the truck driver's zombified dog, he is rescued by Claire. Leon and Claire retrieve weapons from the bodies of the police officers and encounter Ben locked up in a cell, but a zombie inmate bites him before he can escape.
Zombies swarm the police station, and Irons, Claire, and Leon escape to the Raccoon City Orphanage to look for a secret Umbrella tunnel. A licker kills Iron and almost gets Leon too, but he is rescued by Claire. Lisa Trevor, Claire's friend when they were both living at the orphanage, recognizes Claire grown up and gives her and Leon the keys to the hidden tunnel. They discover the illegal research lab where Umbrella Corporation scientists experimented on children like the Ashford Twins.

Wesker encounters Dr. Birkin. In the ensuing scuffle, Birkin shoots Wesker, who shoots him back, and kills his wife in defense. Jill shoots him when Wesker hesitates to pull the trigger on their child Sherry. Before he dies, Wesker warns them that the Umbrella Corporation is going to destroy the city and that their best chance is to escape in the underground train before then. Chris discovers that to survive, William has injected himself with the experimental "G-Virus." Leon, Claire, Jill, Sherry, and Chris escape to the train. The train stops when the Umbrella Corporation destroys Raccoon City and the Spencer Mansion, allowing Birkin, now a horrific, mutated monster, to assault the group as the creature tries to abduct and murder Claire. Leon shoots the monster with a rocket launcher, killing it for good and saving Claire. The five survivors exit the train tunnel, leaving Raccoon City behind, as the Corporation states that there were zero civilian survivors in the aftermath of the destruction.
In a mid-credits scene, Wesker regains consciousness in a body bag, unable to see in the blackness. A mysterious figure then hands him sunglasses and reveals herself as Ada Wong.
