1666

Deena experiences a vision of the events of 1666 from the perspective of Sarah Fier after bringing back her severed hand to the rest of her corpse. She lives in the town of Union with her brother Henry and her father George, the original settlement divided into Shadyside and Sunnyvale.

Sarah and her friends Lizzie and Hannah Miller meet a reclusive widow to gather potent berries for a party, where Sarah finds a book of black magic. The group arrives at the party, and Caleb harasses Sarah and Hannah. The two flee and share an intimate moment, unknowingly seen by Mad Thomas. Pastor Cyrus Miller begins to act strange the next day, as the town's supply of water and food is poisoned. Sarah wonders if she is responsible for the town's bad luck and confides in Solomon Goode. The city then discovers that Pastor Miller has murdered twelve children in the chapel, including Henry and Constance. Pastor Miller attacks Sarah before Solomon kills him. That night, the town holds a meeting during which they decide that witchcraft is the cause of their troubles, and Caleb claims that Hannah and Sarah are the guilty witches. The townspeople capture Hannah while Sarah escapes, and the town declares that they will execute Hannah at dawn.

Sarah sneaks into the chapel that Hannah is confined in, and they profess their love for each other. Sarah vows to acquire the widow's book of black magic to deal with the devil and save Hannah, but she finds the widow murdered and the book missing. Sarah is ambushed by men hunting for her and flees to hide in Solomon's house. She finds tunnels under his home, as well as the widow's book and a ritual. Solomon reveals he used the book to make a deal with the devil, offering Pastor Miller for possession in exchange for wealth and power. Sarah rejects his offer to share the devil's power with her. The two fight, and Miller cut off Sarah's hand in the struggle. She escapes to the chapel only for the townspeople to capture her. Sarah convinces the town during Hannah and her execution to spare Hannah's life by proclaiming she is the witch. She swears vengeance to Solomon before the townspeople hang her. Later, Isaac, Lizzie, Hannah, and Abigail grieve Sarah and properly bury her body.

1994

In 1994, Deena realizes that the firstborn of each Goode family generation repeats the ritual begun by their ancestor Solomon, continuing the Shadyside curse while Sunnyvale prospers. Sheriff Nick Goode finds Deena and Josh, but the two escape in Nick's car and arrive at Ziggy's house. Deena, Josh, and Ziggy deduce that killing Nick will end the curse. The group concocts a plan with the help of the mall janitor Martin to lure Nick to the mall and set traps to allow Shadyside murderers to kill him.

The group traps the murderers, and Ziggy pours a bucket of Deena's blood over Nick, causing the killers to attack him. Nick escapes into the tunnels. Deena and a possessed Sam to follow him while the others fight off the killers. Sam shoots Deena, but she temporarily breaks Sam out of her possession before incapacitating her. Nick nearly kills Deena before she exposes him to the pile of beating organs in the tunnel, giving him a vision of all the victims of the killers. Deena kills him, breaks the curse, and reunites with Sam. The Goode Family is later exposed for their actions, Josh meets his online friend in person, Ziggy reunites with Mrs. Lane, and Deena and Sam have a picnic date at Sarah Fier's grave.

In a mid-credits scene, an unknown person takes the widow's satanic book from the tunnels.
