Eight years after the first film's events, blind Navy SEAL veteran Norman Nordstrom lives with 11-year-old Phoenix and Shadow, his Rottweiler, in Detroit. Norman told Phoenix that her birth mother died in a fire that consumed their old house.

Veteran Army Ranger and Norman's only connection to society Hernandez convinces Norman to let Phoenix go with her on an errand in the town so she can have a bit of time out of the house. A biker is scared away by Shadow while trying to abduct Phoenix. His gang arrives at Norman's by following Hernandez's van, where they wait and kill Hernandez after she drops off Phoenix. They then lure away and kill Shadow. The gang breaks into Norman's house to kidnap Phoenix after Norman goes outside to look for Shadow. A struggle ensues between Norman and the band when the gang leader, Raylan, tells Phoenix that he is her birth father, confirming it by showing they both have a Mallen streak.

It turns out that Phoenix's house burned down after a meth lab explosion in the basement, and Raylan has been in jail for eight years. Norman took Phoenix home to take the place of his dead daughter, whom he found unconscious in the wreckage. When Raylan was released he saw Phoenix alive when she left flowers at her mother's memorial. A gangster chloroforms Phoenix. Raylan commands his dog to attack Norman, but he tricks the dog and traps it in the attic. The gang leaves with Phoenix after setting the house on fire. Norman and the dog escape and they befriend each other before the dog leads Norman to the gang's hideout in a hotel room.

Raylan reveals Phoenix's real name Tara at the hotel and introduces her to her mother Josephine, who is alive but terminally ill in a wheelchair. She explains that while she was cooking meth she accidentally triggered the explosion which poisoned her internal organs. Tara's parents abducted Phoenix solely for an organ transplant for her mother. Tara will be conscious while her parents remove her heart due to the lack of facilities and drugs. Luckily, a power blackout prevents the procedure. Norman ambushes Raylan's men, and he picks them off one by one in the darkness except for Raul, who having learned Raylan's real intention for kidnapping Tara changes sides. Norman kills the surgeon with smoke bombs and attacks Raylan. One of Raylan's stray gunshots kills Tara's mother during the fight, causing her wheelchair to move and drag along a handcuffed Tara. Norman gouges Raylan's eyes and leaves him dead, while Tara, still handcuffed to the wheelchair, escapes by cutting the arms off Josephine's corpse just as her body falls into the pool carrying her wheelchair and Tara with it.

Norman, badly wounded, confirms to Tara that what her father was saying was the truth. He confesses his crimes, including rape and murder, and tells Tara to flee to safety. Raylan appears and stabs Norman, but is fatally stabbed by Tara, resulting in Raylan falling down the empty pool to join his wife's corpse. Tara tries to help Norman, claiming she can save him, to which he replies she already has, before he apparently succumbs to his wounds. Tara leaves and goes to a children's home she had seen earlier. She introduces herself as Phoenix to a group of children playing.

In a post-credits scene shows Raylan's dog approaching Norman's seemingly dead body. The dog licks Norman's fingers, hinting that he may still be alive.
